How to Generate and Print Notes

Overview

How and when to use all the notes in notes tab located on the Admin. row.

Goal

To effectively generate and print notes with Phoenix Ortho.

Generating and Printing an Office Note

Click Search>Search for Patient>Double Click to Open Patient Chart

Open visit date by left clicking the plus sign next to the date of service

To know you need to print an Office Note look in the History Tree. If the patient has body part Exam in History Tree, you will select Office Note to generate. This patient has a Cervical body part exam.

To generate the note>Click Notes on Admin. Row>Click Office Note>A word document will generate and be saved in the patient chart

To print the note: Double Click Office Note in History Tree to open word document>Click File in Microsoft Word>Click Print

Generating and Printing a Follow-up Note and Follow-up Note with History

Click Search>Search for Patient>Double Click to Open Patient Chart

Open visit date by left clicking the plus sign next to the date of service

To know you need to print an Follow-up Note look in the History Tree. If the patient has Follow Up exam type in the History Tree you print a Follow Up Note or Follow Up Note with History. The below patient was seen for a Follow Up- Knee today.

To generate the note: Click Notes on Admin. Row>Click Follow-up Note>A word document will generate and be saved in the patient chart

 

Note: The difference between a Follow-up Note and a Follow-up Note with History is the Follow-up Note with History contains all of the patient History.

 

To print the note: Double Click Follow up Note in History Tree to open word document>Click File in Microsoft Word>Click Print

How to Generate and Print a PT/OT Note

Click Search>Search for Patient>Double Click to Open Patient Chart

To know you need to print an PT/OT Note look in the History Tree. If the patient has PT/OT Exam type in the History Tree a PT/OT note should be printed. The below patient has a PT Exam-Hand for today's visit.

To generate the note: Click Notes on Admin. Row>Click PT OT Note>A word document will generate and be saved in the patient chart in the History Tree.

To print the note: Double Click PT OT Note in History Tree to open word document>Click File in Microsoft Word>Click Print.

Types of Notes you can Generate

PDF Forms: This is where custom PDF's generated by your clinic are housed.

Create Work Release: This is where you can create and generate a work release to print for your patient.

School Athletics: This is where you can create and generate a school athletics to print for your patient.

School Absence: This is where you can create and generate a school absence to print for your patient.

Office Note: To be created when a body part exam is done for the patient's visit.

Patient Summary: This is used to provide a patient a summary of their visit. This includes all of the tabs under the patient summary and is also what is available on the patient portal.

Consultation Report: This note generates a summary of the patient's visit and populates a salutation for the referring provider listed in the patient demographics.

Referral Out: Used if you are going to refer a patient to take over care on this patient. If used it will calculated in the denominator of your Clinical Summary, Referral Out Meaningful Use Measure.

HIPAA Notice: This note is not used.

Follow-up and Follow-up w/ History: To be created when a follow-up exam is done for the patient's visit.

Pre-op Note: This note generates a Pre-op Note and will populate a body part exam and physical exam.

Post Op | Fracture Care: Not a note used anymore.

PT OT Note: To be created when a PT OT exam is done on for the patient's visit.

X-ray Note: This notes generates a copy of the X-ray and findings for that date of service.

Dictate Note: Note that pulls today's visit items and can be added to by the provider with Dragon or typing. Example: If the provider needs to send a letter to a claims manager.

Provider Certification Queue: Used for a provider to sign off on PA or Nurse Practitioner visits.

Clinical Summary: Not used anymore.
